You can refer yourself to our office or your family physician or other referring physician can also refer you.

A referral form is helpful, but you can also be referred with a simple telephone call.  referral forms help our staff better understand the history of your condition. If you have a referral form completed by a doctor, it can be emailed or faxed to our office.  Referral forms can be downloaded and printed by clicking the above blue button titled “Download a PDF Of Our Referral Form”.

Any patient with chronic tension type headaches, frequent migraine headaches, post traumatic headaches, chronic neck pain, whiplash and facial pain, whose lives are adversely affected by their chronic pain condition, or those patients that require constant analgesic consumption to manage their pain would benefit from a referral.

All medical consultations and treatments including drug and dietary counseling, nerve blocks, trigger point injections are fully funded by O.H.I.P.

Acupuncture, Myofasical, are covered by third party insurance and motor vehicle accident benefits.
